Sometimes you can't keyboard down to the bottom-most choice even though the mouse is far away and not lined up with anything. In those cases I find it helpful to bring the mouse over, spin my scroll wheel back and forth a bit, and try the keys again.And inexplicable dialog choosing sometimes too. Keyboard or mouse, the bottom option of a conversation sometimes just will not be chosen until trying several times. It will instead skip to the one above.
Yeah, there is a horrific mouse/keyboard desync issue plaguing the entire interface. It blows my mind that something like that made it through QA—it is easily the most visible and most annoying bug in the game.I bet I can tell you why it happened, too, and why they were too lazy to fix it: because of the way the interface works, when you go up/down with the keyboard, the list moves up or down for every keypress, always keeping your active selection 'centered' (well, it's not actually centered because the menus inexplicably have a bottom of the screen bias, but that's a whole other issue). The 'problem', as it were, would be that if you highlighted something different with your mouse, that centering would be lost.So they decided to make the mouse and the keyboard able to have different active selections. It can be nearly impossible to tell which one is actually active and which one isn't, and the entire thing basically doesn't work. And to make matters worse, sometimes the mouse just stops highlighting in the menus at all. It's just so very, very bad.
